- Duly Signed KYC form with AML (Anti Money laundering) documents e.g. Pan Card, Electricity bill etc.
- Supporting medical certificate from treating doctor; Attested copies of Indoor case papers.
- Attested copy of First Information Report(FIR)/Medico Legal Certificate(MLC)
- Investigation reports supporting the diagnosis
- Itemized breakup of costs
- Attested copy of Hospital registration certificate.
- Settlement letter from other insurers
