  • Sometimes health insurance policies might require you to undergo a few medical tests. You will be required to submit the medical reports detailing the results of these tests as well.

The documents needed for a claim are different than the ones needed for registering a claim. If you go for a cashless health insurance , you need not worry about submitting any documents to the insurance company. The network hospital, where you received the treatment will send all the required documents as well as details of your treatment to your health insurance company. However, in case you opt for claim settlement by reimbursement, you will need to submit the necessary documents to your insurance company, by collecting them from the hospital where you received the treatment. The insurance company will verify all the documents submitted by you and the claim amount will be directly deposited in your bank account.   • Insurance company's claim form, duly filled and signed by you
  • Discharge card
  • Written consultation from the doctor with receipts
  • Your hospital bills, which have to be stamped and signed by the hospital authorities
  • X-ray films and other test results such as blood test, urine test etc.
  • Medicine bills
  • Other relevant documents relating to the cause of treatment
